The Alzheimer’s Association
The Alzheimer’s Association, a national network of chapters, is the largest national voluntary health organization dedicated to advancing Alzheimer's research and helping those affected by the disease. Having awarded $136 million in research grants, the Association ranks as the top private funder of research into the causes, treatments, and prevention of Alzheimer’s disease. The Association also provides education and support for people diagnosed with the condition, their families, and caregivers. The Association can be contacted at www.alz.org.

The American Academy of Pediatrics
The American Academy of Pediatrics (AAP) and its member pediatricians dedicate their efforts and resources to the health, safety and well-being of infants, children, adolescents and young adults. The AAP has approximately 57,000 members in the United States, Canada and Latin America. Members include pediatricians, pediatric medical subspecialists and pediatric surgical specialists. More than 34,000 members are board-certified and called Fellows of the American Academy of Pediatrics (FAAP).

The mission of the American Academy of Pediatrics is to attain optimal physical, mental and social health and well-being for all infants, children, adolescents and young adults. The American Diabetes Association
The American Diabetes Association is the nation's leading nonprofit health organization providing diabetes research, information and advocacy. The mission of the organization is to prevent and cure diabetes and to improve the lives of all people affected by diabetes. To fulfill this mission, the American Diabetes Association funds research, publishes scientific findings, provides information and other services to people with diabetes, their families, health care professionals and the public. The American Geriatrics Society
The American Geriatrics Society (AGS) is the premier professional organization of health care providers dedicated to improving the health and well-being of all older adults. With an active membership of over 6,000 health care professionals, the AGS has a long history of effecting change in the provision of health care for older adults. In the last decade, the Society has become a pivotal force in shaping attitudes, policies and practices regarding health care for older people. The AGS can be contacted at www.ags.org.

Family Voices
Family Voices is a national, grassroots clearinghouse for information and education concerning the health care of children with special health needs. We stay on top of public and private sector health care changes that affect our children and families through the collective efforts of our families: a volunteer Coordinator in every state; 10 Regional Coordinators; and a small staff working in several locations around the country. Together, we share the expertise and experiences of families from around the country with state and national policymakers, the media, health professionals, and other families. We work in public and private hospitals, public health programs, in state capitals, in Washington, DC, serving on boards and task forces, working in partnership with health professionals and policymakers, bringing the family perspective to policy discussions and decisions. There are almost 40,000 Family Voices members - families of children with special health needs and friends and professionals who know and love our children. The National Alliance for the Mentally Ill
The National Alliance for the Mentally Ill (NAMI) is a nonprofit, grassroots, self-help, support and advocacy organization of consumers, families, and friends of people with severe mental illnesses, such as schizophrenia, major depression, bipolar disorder, obsessive-compulsive disorder, and anxiety disorders. NAMI can be contacted at www.nami.org.

The National Chronic Care Consortium
The National Chronic Care Consortium (NCCC) is a strategic alliance of individuals and organizations across industry segments and professions committed to chronic illness care reform. We engage people in practical, collaborative methods to improve care for America's largest, highest-cost, and fastest-growing customer group. We work to improve care under current market conditions and seek to change payment methods, health policies, management systems, and clinical interventions where they impede the ability of consumers to get the care they want and of plans, programs, and providers to offer effective care. The Robert Wood Johnson Foundation
Established in 1972, The Robert Wood Johnson Foundation, based in Princeton, N.J. is the largest philanthropy devoted exclusively to health and health care in the United States and provides funding for Partnership for Solutions. The Foundation concentrates its grantmaking in four areas: assuring that all Americans have access to basic health care at reasonable cost; improving care and support for people with chronic health conditions; promoting healthy communities and lifestyles; and reducing the personal, social and economic harm caused by substance abuse — tobacco, alcohol, and illicit drugs.
